Member Service Certification
IRS regulations regarding a 501 ( C ) 19 Veterans non-profit organizations require that a Member must be a War Veteran, to allow donations to be tax deductible. A Member or War Veteran is required to have served from:
12/07/1941 - 12/31/1946 6/27/1950 - 1/31/55
2/28/1961 - 5/7/1975, in the Republic of Viet Nam during that period;
7/5/64 through 5/7/75 8/2/1990, and ending on the date prescribed
by Presidential Proclamation or by law.

Supporting documentation is any government document issued stating time in service, or providing evidence of service
in a combat zone or time of war, that fall within the above IRS service periods. Examples are, but not restricted
to DD-214, military combat awards, military awards showing date of award, military orders, etc.
I attest and affirm that the above information is true and accurate to the best of my knowledge, and understand that supporting documentation must be provided to Iwo Flags (if challenged by the IRS) and upon request. Failure to meet any requirement or produce documentation, as needed, will be cause to remove the Member from Iwo Flags with all
dues and donates retained by Iwo Flags.
